What to Know Before Your First Visit

The eye is one of the most complex parts of the body. Our eye care experts will conduct a thorough examination in order to properly diagnose any eye conditions or diseases and create a personalized treatment plan. You should plan to spend up to 2 hours at our office for your first visit or for any type of speciality exam. Please bring sunglasses, which you will need to wear if your pupils are dilated for your exam. To protect your identity we require a picture ID for all patients or parents of minors.

Patient Portal

As a patient at our practice, you can use our Patient Portal to schedule appointments, request prescription refills, and communicate with your providers. Emergency Eye Care

The eye doctors at CVP Physicians Dayton provide medical coverage 24 hours a day for our established patients. In the event of an emergency after regular hours, please call (937) 320-2020 and your message will be forwarded to our physician-on-call.
